I went to this place. It was my first time with my dog. The receptionist gave me a form to fill out while I was sitting in the examination room. It was asking permision to use my information according to an electronics act. I asked to read the electronics act and they told me they dont have a copy of it. I told them unless I can see the act im signing permission to use my info i cant sign. She left the room,told the vet, came back and said i didnt have to sign it. When the vet came into the room he told me if i dont sign it, he isnt vaccinating my dog and to leave his office.I dont sign anything giving permission to release my information unless i know what its used for. This office will ask you to sign the form but not have a copy of "The electronics act" for you to read, expect you to sign the realease of information form without knowing whats done with your information, and the vet himself will tell you to leave his office if you dont sign his release of information form. If the money i was going to pay him for the vaccines wasnt as important as this release of information form, then you can bet its a big money making form for him. A release of personal information form thats a money maker is a form NOT TO SIGN.
