Traditions Health Hospice

Traditions Health is a hospice care provider serving Allen and its surrounding communities. We provide skilled nursing, physical therapy, occupational therapy, counseling, and more. Our hospice team is available 24 hours a day, 7 days a week, and includes physicians, skilled nurses, medical social workers, volunteers, and chaplains who provide patient-centered care during the final phases of a patient’s disease process.
