Our mission is to provide integrated primary care services for all people ages 18 to 118, with an emphasis on geriatrics. Our vision is a healthy community of complete physical, mental, and social wellbeing. Our core services include primary care, geriatrics, mental health integration, substance misuse treatment and in-house phlebotomy services. Our providers are available 24/7 through our answering service and we're happy to accept Medicare, Medicaid and commercial insurance plans.
