Post-hospital and rehabilitation guests at Aperion Care Forest Park receive 24-hour skilled nursing care, as well as therapy available 7 days per week. Our spacious, modern facility features an innovative Congestive Heart Failure (CHF) program, Bridge to Health, with a cardiologist doing weekly rounds in-house.
